Summary
The Primary Care Provider (Nurse Practitioner/NP) provides comprehensive clinical care - consultation - and patient management in support of the VA mission. They practice to the full extent of their training and certification - follow defined clinical privileges - and hold prescriptive authority for their assigned patient panel or program.

- The Primary Care Provider (NP) is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
- Duties of the position include are but not limited to: Manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develop and implement individualized plans to achieve optimal population health and disease management
- Demonstrate leadership and work collaboratively with Primary Care Team and other interdisciplinary team members
- Order and interpret labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiate referrals and consultations to specialty care services
- Provide education to patients and their families
- Assess and adjust the plan of care as needed to achieve optimal patient outcomes

- Influence and model the delivery of evidence-based health care - demonstrate expertise in the ability to manage a patient panel - population - or program
- Initiate and lead interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel - population - or program
- Acquire knowledge and skills - then disseminate acquired information within and/or outside the medical facility
- Initiate and lead interdisciplinary teams developing systems that positively impact outcomes at the panel - population - or program
- Demonstrate effective communication skills and professional behavior - promoting cooperation and teamwork
- Meet the needs of customers while supporting the medical center and service missions
- Communicate and treat customers (patients - visitors - volunteers - and all medical center staff) in a courteous - tactful - and respectful manner
- Provide the customer with consistent information according to established policies and procedures
- Handle conflict and problems in dealing with the customer constructively and appropriately
- Other duties as assigned
- VA offers a comprehensive total rewards package: VA Nurse Total Rewards Pay: Competitive salary - regular salary increases - potential for performance awards Paid Time Off: 50 days of paid time off per year (26 days of annual leave - 13 days of sick leave - 11 paid Federal holidays per year) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory Work Schedule: Monday to Friday - 0800-1630 Telework: The Veterans Health Administration - including VISN 19 and the Salt Lake City Health Care System - will comply fully with the intention and instructions contained in the \"Return to Office\" Executive Order
