Summary
This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Duties
- NOTE: This is a multi-discipline recruitment that will result in the hiring of one of the following disciplines: Nurse Practitioner or Physician Assistant
- All Physician Assistants applicants should apply to posting: CBTD-12983890-26-CB There are 2 Nurse Practitioner positions located in Primary Care Service
- One Nurse Practitioner position is located at the Manchester VA NH location
- One Nurse Practitioner position is located in Women's Health at the Portsmouth NH VA CBOC location
- Both positions are full-time at 40 hours per week
- Included but not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes Exemplifies the ICARE core values of-Integrity - Commitment - Advocacy - Respect - and Excellence Work Schedule: Full time - Monday- Friday 7:30am-4:00pm Compressed/Flexible: Not Authorized Telework: Not Authorized Virtual: This is not a virtual position
