Summary
This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Duties
- This vacancy will be filled with either a Nurse Practitioner or a Physician Assistant
- This position is located at the Northwest VA Clinic - Surprise - AZ
- The Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
- The NP provides advanced clinical practice - consultation - and management.The NP practices to the full extent of their education - training - and certification
- Core functions are outlined in the clinical privileges that correspond with this position
- The NP has prescriptive authority for a panel of patients or program as indicated by their position
- FUNCTIONS OF ASSIGNED DUTIES: The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
- Duties include but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

- Clear and Timely Communication: Applicants will receive clear communication at key milestones - including referral - tentative offer - and onboarding instructions
- After meeting pre-employment requirements - candidates will receive a final job offer and start date
- Onboarding tasks may continue after starting - with HR ensuring timely completion for a smooth transition into VA service
- If selected - applicants should be prepared to come onboard within 30 days of the announcement closing
