Summary
This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Duties
- On the Nurse Practitioner NG 61 pay scale The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) leads facility efforts in population management for health promotion and disease-management initiatives
- They collaborate with primary care teams - specialty services - and community partners to strengthen preventive care delivery and support evidence-based wellness programs
- The NP manages training and coaching activities for clinical staff and applies data - performance metrics - and structured action plans to monitor outcomes and drive continuous quality improvement
- They ensure effective coordination of behavioral health and wellness activities across primary care - aligning programs with facility priorities and identified population needs
- The NP practices in Primary Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
- The NP provides advanced clinical practice - consultation - and management
- The NP practices to the full extent of their education - training - and certification
- Core functions are outlined in the clinical privileges that correspond with this position
- The NP has prescriptive authority for a panel of patients or program as indicated by their position
- The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
- Major duties and responsibilities of the position: Clinical Care Duties Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
- Leadership and Advocacy Leads efforts to identify key challenges and priority areas for improvement - using data and performance metrics to guide decision-making and advocate for effective interventions
- Serves as a primary point of coordination for system-wide preventive-health initiatives and ensures that facility teams are informed - prepared - and engaged in implementation
- Builds and maintains collaborative relationships with internal and external partners to support Veteran wellness and expand the reach of preventive services
- Program Development and Implementation Oversees the design - implementation - and evaluation of health promotion and disease-management programs - ensuring they are evidence-based and responsive to local Veteran needs
- Develops structured action plans - workflow processes - and standardized tools to support consistent delivery of preventive-care services across primary care settings
- Monitors program performance using data dashboards - quality measures - and outcome reports - and adjusts strategies to ensure continuous improvement
- Ensures effective communication and coordination across care teams so that preventive-health services - behavioral-health interventions - and wellness activities are implemented consistently throughout the facility and affiliated clinics
