Summary
The John D. Dingell VA Medical Center is recruiting for a Nurse Practitioner- Palliative Care. The NP practices to provide a full range of services in outpatient/inpatient settings emphasizing quality - timeliness & the advancement of the mission. The NP provides advanced clinical practice - consultation & management. The NP practices to the full extent of their education - training - & certification.
Duties
- The NP has prescriptive authority for a panel of patients or program as indicated by their position or assigned by their supervisor
- The major duties include - but are not limited to: Independently manages or in collaboration with attending physician(s) - a panel of patients with acute and chronic health conditions with emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with the interdisciplinary team
- Initiates referrals and consults to specialty services
- Provide End-Of-Life care and symptom management to all patients in an inpatient and outpatient setting
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
- Provider is committed to delivering compassionate - timely and Veteran-centered care through multidisciplinary palliative care teams
- These teams support Veterans in VA hospitals - community living centers (CLCs) and even remotely via telehealth services
- Provides medical services to veterans on an Outpatient - Inpatient or assigned by the supervisor as needed at the John D Dingell VAMC under GEC service
