Summary
The Nurse Practitioner (Home Based Primary Care) practices in Geriatrics and Extended Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission. The Nurse Practitioner (NP) provides advanced clinical practice - consultation - and management.
Duties
- Major duties include but may not be limited too: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with the interdisciplinary team
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

- Provides care within the GEC/Home Based Primary Care Program including health promotion - health prevention - geriatric care - primary care - and palliative care
- Delivers competent care in the home of Veterans with complex needs - utilizes interventions to maximize health capabilities and reduce unnecessary urgent care and emergency room visits - per Veteran's goals of care
- Makes home visits - Veteran Video Connection (VVC) visits and telephone visits to Veterans in the HBPC program Preferred Experience: Two years experience as a Nurse Practitioner - Prescriptive Authority - and an Active Valid state driver's license
