Work Type: Full Time
Shift: Shift 1/8:00:00AM to 5:00:00AM
Minimum to Midpoint Pay Rate:$35.04 - $47.30 / hour
Summary
The Inpatient Heart Failure Nurse Navigator is a registered professional nurse with advanced clinical expertise who serves as a key patient advocate and educator, guiding individuals with hear failure through their healthcare journey. The Heart Failure Nurse Navigator works closely with physicians, nurses, and other healthcare professionals to optimize disease management and reduce hospital readmissions. Role responsibilities include supporting the development, implementation and maintenance of the heart failure program through assessment and treatment of heart failure patients, development and support of clinical practice guidelines, collection and analysis of heart failure data, compliance and monitoring with heart failure guidelines, program initiatives and regulatory requirements, performance improvement, staff and patient education. The Heart Failure Nurse Navigator serves as a clinical role model, educator, and liaison for the heart failure team, hospital staff and community collaborating with the Advanced Heart Failure (AHF) Medical Director, Program Coordinator, and nursing leadership to facilitate and promote improved patient outcomes.
Requirements
Education:Bachelors in nursing required, Masters preferred.
Experience: Minimum of 3 years' experience in related field. Demonstrated teaching experience in the clinical and community setting required.
License: Current State of Florida RN licensure
Certifications: American Heart Association Healthcare Provider Basic Life Support (BLS) required.
Other:Strong clinical background in an acute care setting. Strong customer focus. Excellent written and verbal communication skills. Proficiency in use of electronic medical records. Ability to organize and lead meetings as well as present in groups. Ability to work independently, delegate and multi-task in a fast-paced environment. Strong problem solving and decision-making skills.
