Location: HealthPark Medical Center -9981 So HealthPark Circle Fort Myers FL 33908Work Type: Full Time
Shift: 0/8:00:00 AM to 4:30:00 PM
Minimum to Midpoint Pay Rate: $30.27 - $40.86 / hour
Offering Relocation Reimbursement of $8,500 plus 3 months of housing allowance $1800 per month
The Heart Failure Nurse Navigator provides education, care coordination, and support for patients living with heart failure. This autonomous role involves triaging patients, managing a personal caseload, and collaborating across the care continuum to improve outcomes and close gaps in care.This is your opportunity to grow and thrive while guiding patients through complex cardiac care.

- Nurse Navigator- Heart Failure Unit at Health Park Medical Center
- Nurse Navigator- Advanced Heart Failure Ambulatory
- Nurse Navigator- Heart Care Transition Clinic
What Youll Do:
- Educate patients and families on heart failure, medications, lifestyle, and self-management.
- Triage and prioritize patients, scheduling follow-ups based on urgency and need.
- Identify gaps in care and collaborate with interdisciplinary teams to optimize outcomes
- Build relationships across inpatient, outpatient, and community care settings
- Stay current on guideline-driven heart failure care and best practices.
Requirements:
- RN license in Florida; BSN preferred.
- cardiac nursing experience is a must; heart failure, palliative care, or transplant experience preferred.
- BLS required; cardiac specialty certifications preferred.
- Strong patient education, communication, and care coordination skills.
- Able to work autonomously, prioritize effectively, and advocate for patient needs.
