Summary
This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Duties
- **This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until July 10 - 2026
- The initial cut-off date for referral of eligible applications will be May 25 - 2026 with subsequent cut-off dates every 2 weeks
- Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available
- NOTE: This position is based out of the EUREKA VA Outpatient Clinic - duties entail traveling to the homes of Veterans living in the surrounding area
- The Home Based Primary Care Nurse Practitioner (HBPCNP) serves veterans with complex - chronic - and disabling diseases who have difficulty traveling to VA facilities for care
- HBPC delivers comprehensive - interdisciplinary primary care right in the veteran's home
- Given the rural and geographically isolated setting of Eureka - the HBPCNP plays a critical role in ensuring access to timely - continuous primary care services
- The position requires strong clinical judgment - adaptability - and a commitment to serving Veterans in an underserved region while upholding VA standards of care - performance metrics - and regulatory requirements
- The incumbent independently manages a panel of patients with acute and chronic health conditions - with an emphasis on health promotion and disease prevention - using evidence-based practice in the home of Veterans
- A valid Driver's License and insurance are required
- Major duties and responsibilities of the position: Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with the interdisciplinary team
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
