Summary
The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Home Based Primary Care (HBPC) within the Geriatric Extended Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission. The NP provides direct Veteran care and care coordination - as it pertains to Veterans and their care within the HBPC Program.
Duties
- The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
- Core functions are outlined in the clinical privileges that correspond with this position
- Major duties include - but are not limited to: Provide advanced clinical practice - consultation - and management
- Utilizes interventions - which maximize health capabilities and reduces unnecessary urgent care and emergency room visits - per Veteran's goals of care
- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Makes home visits to Veterans in the program
- Collaborates in the development - implementation - evaluation - and revision of policies - procedures - and/or guidelines
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with the interdisciplinary team
- Orders and interprets labs - tests - and diagnostic studies
- Supports colleagues and other nurses through HBPC knowledge sharing to provide safe - quality nursing care
- Evaluates outpatient outcomes of evidence-based decisions and practice changes for HBPC individuals - groups - and populations
- Responsible for the documented HBPC outcomes at the program or service level
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Responsible for case management - assessment - triage - Veteran education - resource coordination - medication and treatment coordination - and care planning
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
- Prescriptive authority for a panel of patients or program as indicated by their position and state law
