Summary
This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Duties
- The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Primary Care Service Residency Program Clinical Faculty to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
- The NP provides advanced clinical practice - consultation - and management
- As an APN - you will: The primary clinical faculty is assigned to the residents for the entire year and provides clinical supervision - guidance - and mentoring
- The clinical faculty will be responsible for structured and formal evaluations of the residents' competency - performance - and progress
- The resident's continuity of care clinic is under the supervision and oversight of their clinical faculty member
- The residency program director will meet regularly with the clinical faculty to evaluate progress and ongoing professional development needs
- The clinical faculty will be available to the residents as needed for mentorship and guidance
- Residents - clinical faculty - and the program director will meet weekly as a group to provide support and guidance
- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
