Summary
The Cardiology Advanced Practice Nurse (APN) - Certified Nurse Practitioner (NP) serves within the National CARDIO-VET Network - delivering a comprehensive range of cardiology care aligned with the mission of the Department of Veterans Affairs. The APN provides advanced clinical practice - expert consultation - and ongoing patient management while ensuring high-quality - timely - and Veteran-centered services.
Duties
- Major duties and responsibilities of the position VN-0610-II and VN-0610-III include but not limited to: Independently manages a panel of patients with cardiovascular conditions with an emphasis on cardiovascular disease management using evidence-based practice
- Develops and implements an individualized plan to achieve optimal cardiovascular care management
- Demonstrates leadership and works collaboratively with the interdisciplinary team
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
