Summary
The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in the Cardiology Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice - consultation - and management.
Duties
- The Cardiology APN is primarily responsible for providing and coordinating the care of patients referred for structural heart and other advanced procedures within Cardiology
- The APN will be an integral member of a growing cardiology program that performs a range of structural heart procedures - including transcatheter heart valve replacement/repair and left atrial appendage occlusion
- The APN/Cardiology will evaluate patients - develop comprehensive plans - and implement and coordinate cardiac care in clinical and telehealth settings
- The role requires independent and autonomous decision making based on knowledge of nursing and cardiovascular diseases
- The provider will be expected to coordinate care with referring physicians - cardiology and cardiac surgery - anesthesia - hospital medicine - and cardiac rehabilitation
- Aspects of the role will also work with other APN providers to support clinical care and coordination for Cardiology patients who need clinical management related to General Cardiology - Heart Failure - and Electrophysiology
- The NP practices to the full extent of their education - training - and certification
- Core functions are outlined in the clinical privileges that correspond with this position
- The NP has prescriptive authority for a panel of patients or program as indicated by their position
- FUNCTIONS OF ASSIGNED DUTIES: The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
- Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with the interdisciplinary team
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
