Description

Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. As a Federally Qualified Health Center (FQHC), we provide vital health services to almost 40,000 individuals annually in Marin County. The Clinics regularly receive national awards from the Health Resources and Services Administrations (HRSA). Our Mission is to promote health and wellness through excellent, compassionate care for all.

Working collaboratively with primary care, behavioral health, and other members of the clinical care team, the Complex Care Clinical Supervisor provides clinical expertise and supervision to a team of Case Managers and Associate Case Managers. The Complex Care Clinical Supervisor will work closely with the Director of Complex Care to ensure that patients receive excellent, high-quality case management.  To achieve this, the Clinical Supervisor will provide direct administrative and clinical supervision to Complex Care Case Managers, providing clinical support and oversight for Case Managers. In addition, the Clinical Supervisor role is a patient-facing role that aims to build engagement and trust with our Complex Care patients by ensuring the highest quality of case management support. 

Responsibilities:

  • Works closely with the Director of Complex Care to ensure the delivery of high-quality, person-centered case management across the department.
  • Provides clinical support and oversight to Case Managers/Associate Case Managers and their patients including hospital follow-ups, medication questions/refills, triage, and other actions.
  • Completes new -patient assessments with Associate Case Mangers to ensure quality.
  • Facilitate internal and external case conferencing.
  • Provide program development support (creating workflows, grant participation, program evaluation, etc.).
  • Ensure program reporting requirements and quality measures are met.
  • Conduct home/community visits as needed.
  • Document all Patient interactions and services using MCC’s electronic health record AND secondary database.
  • Attend internal and external meetings (i.e., partners, case conferencing, administrative)
  • Attend relevant trainings such as health coaching and engagement techniques.
  • Provides timely and accurate reporting of data as requested to MCC CM programs.
  • May travel between clinics or meet Patients in the community as needed.
  • Provides frank and considered opinions regarding case management services and quality improvement measures.
  • Other duties may be assigned (including maintaining small caseload).

Qualifications:

Supervisory Skills:

  • This position will spend more than 50% of the time directing the work of Case Managers and Associate Case Managers.
  • Maintains appropriate staff levels and participating in recruitment. This includes working collaboratively with Recruiters and may involve dispositioning candidates utilizing our ATS (ICIMS), interviewing and checking references.
  • Onboards, trains and supports continued development of staff.
  • Manage employee performance, effectively communicate expectations and goals and provide performance feedback and evaluation in a thorough and timely manner.
  • Complete time and attendance requirements for team, approving timesheets, time off requests, and ensure accurate and timely completion for payroll.
  • Ensures a safe, secure, and legal work environment by upholding MCC’s policies and procedures.

Computer Skills:                                                    

  • To perform this job successfully, an individual must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.


Requirements

Education and Experience:

  • RN or equivalent experience in healthcare setting.
  • Experience with administrative/operations management.
  • A minimum of one -two years’ experience (and passion for) providing support to individuals with chronic and complex BH needs, complex medical conditions and/or substance-using individuals highly preferable.
  • Knowledge of state regulations and legal/ethical standards related to patient rights, and client/patient confidentiality required.
  • Training in motivational interviewing highly desired/or willingness to learn. Has clinical experience and expertise service complex care patients; particularly patients experiencing homelessness, trauma, and chronic medical conditions.
  • Has experience providing clinical oversight and guidance to staff.
  • CPR Training Current.
  • Valid Driver’s License.

Required Skills and Abilities:

  • Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Must be able to communicate with patients and partners in Spanish: effectively present information and respond to questions from clients, customers, and the general public in Spanish.
  • Strong preference for a candidate than is able to conduct clinical assessment interviews and case management interventions with patients in Spanish.
  • Ability to write reports and business correspondence in English.
  • Demonstrates experience/initiative in program development and improvement.
  • Demonstrates leadership in the field of Complex Care.
  • Is able to effectively give and receive feedback and demonstrates flexibility and adaptability.
  • Has demonstrated excellent coalition building skills and is comfortable working with community partners including community-based organizations (CBOs), Marin County, medical/BH specialists, and insurance providers.
  • Has excellent communication and organizational skills.
  • Is comfortable working with individuals with complex behavioral health presentations and/or substance use disorders.
  • Has passion for helping individuals navigate a complex system of care.
  • Is a team player and welcomes guidance from multidisciplinary team members.
  • Demonstrates initiative, creativity, and enjoys problem solving.
  • Ascribes to the tenets of person-centered, trauma-informed, housing-first care.
  • Demonstrates compassion and respect for individuals served in all interactions.

Physical Requirements:

  • Prolonged standing and walking.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
  • Will to drive to clinics and other locations in the community.
